Experience:
1 year of ambulatory care experience
Cardiac nursing experience with management of heart failure and/or heart transplant recipients


License:
Valid registered nurse license


Certifications:
AHA BLS


Must-Have:
Ability to float to other facilities


Description:
This position will be responsible for the coordination of care for patients who have heart transplants.
Responsibilities include immunosuppression monitoring, prophylactic antibiotic management, monitoring for organ rejection, monitoring for infection, and other routine patient monitoring.
Preferred experience includes the management of other organ transplant recipients and knowledge of immunosuppressive therapies.
The role includes oversight of laboratory results to detect changes that may need immediate action.
Effective communication with multidisciplinary team members, patients, and families in regard to patient care issues is essential.
Facilitation of patient activities with consulting departments and physicians is required.
The care coordinator will assist other nurses and staff in assessing, planning, implementing, and evaluating care for transplant patients.
Assessment of patient and family educational needs and directing communication of these needs to appropriate services is expected.
This position coordinates all post-heart transplant patient care across the continuum, throughout all phases of the transplant process, per provider orders and protocols.
Candidates must have experience in the management of heart failure and be familiar with care coordination and disease management.
