*Job Description*
Focus Home Health is hiring a Licensed Occupational Therapist (OT) for home health visits in the Peninsula & North Santa Clara Territory. This position offers flexible scheduling, 1:1 patient care, and strong clinical support from an experienced leadership team.

*Responsibilities*

* Perform in-home occupational therapy evaluations and treatments
* Assess patient functional abilities, including strength, coordination, endurance, communication, and ADLs
* Evaluate home environments for safety risks and barriers to independence
* Develop, implement, and update OT plans of care in collaboration with RNs and physicians
* Complete OASIS documentation, progress notes, and re-evaluations in a timely manner
* Supervise Occupational Therapy Assistants (OTAs) as assigned
* Monitor patient progress and report significant changes to the care team
* Obtain physician orders to modify plans of care
* Educate patients and caregivers on adaptive equipment, safety strategies, and independent living techniques

*Qualifications*

* Active Occupational Therapist (OT) license California
* Bachelors degree in Occupational Therapy
* Minimum 1 year of OT clinical experience (home health preferred, not required)
* Strong documentation and time-management skills
* Experience with Home Care Home Base (HCHB) or similar EMR preferred
* Reliable vehicle, valid drivers license, and current auto insurance
