Responsibilities
- Conduct assessments and develop care plans for 18–22 patients on the assigned unit, focusing on current inpatient practices and patient needs.
- Coordinate and monitor services to promote quality, cost-effective outcomes; implement and adjust plans as patient needs evolve.
- Determine medical necessity and appropriateness of admissions using InterQual criteria; support LOS management and patient flow.
- Collaborate with the interdisciplinary team including physicians, nurses, and social workers to plan and execute transitions of care.
- Document patient information and progress in the electronic health record (EPIC); communicate needs with patients and families.

Requirements
- Active RN license in Massachusetts
- BSN degree required
- Minimum 3 years of experience in case management, utilization management, or acute inpatient care; 5+ years preferred
- CCM or related case-management certification preferred
- EPIC experience and InterQual knowledge preferred; willingness to float to other units as needed
