Role Responsibilities

Education Requirements

• MD or DO from an accredited institution.
• Completion of a recognized Family Medicine residency program.

Certifications

• Board Certified in Family Medicine.
• Hold (or be eligible for) an active medical license in Ohio.
• BLS and ACLS certifications are preferred.
