About the Role: Solace Hospice & Palliative Care Inc is looking for a compassionate and dedicated Hospice RN Case Manager to join our team in Saint Charles, IL. Responsibilities:

• Manage a caseload of hospice patients, conducting comprehensive assessments and developing individualized care plans

• Perform skilled nursing visits in patient homes, skilled nursing facilities, and other care settings throughout the Saint Charles area

• Coordinate interdisciplinary team (IDT) care, collaborating with physicians, social workers, chaplains, and aides

• Attend every-other-week Interdisciplinary Team (IDT) meetings (in person or via phone/video) to review patient plans of care, evaluate progress, and collaborate on care coordination

• Monitor and manage pain and symptom control, adjusting care plans as patient needs evolve

• Educate patients and families on disease progression, comfort care, and end-of-life planning

• Maintain accurate and timely clinical documentation in compliance with hospice regulations and Medicare guidelines

• Participate in the scheduled on-call rotation to provide coverage for urgent patient and family needs after hours and on weekends

Requirements:

• Active RN license in the state of Illinois, in good standing

• Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred

• Strong knowledge of Medicare hospice regulations and care standards

• Excellent clinical assessment, pain management, and symptom control skills

• Exceptional communication and interpersonal skills with patients, families, and care teams

• Ability to work independently, manage time effectively, and prioritize a dynamic caseload

• Valid driver's license and reliable transportation for field-based visits
