Hospice Clinical Liaison - RN

Clinical Patient Family Liaison, Registered Nurse Location: Glendale and Los Angeles, CA Salary: $50 - $60 / hour Schedule: M-F Overview Reimagine your career in hospice with a purpose-driven, collaborative culture that prioritizes exceptional care for patients and staff. You will support families and patients through compassionate, interdisciplinary care. Qualifications One (1) year of RN experience in the local market with strong knowledge of governmental home health regulations, Medicare requirements, and care plan development techniques. Previous medical sales experience preferred. Licensed RN currently licensed in the state of agency operation. Licensed driver with an insured automobile in good working order. Responsibilities Provide community and provider in-service education in assigned territory. Maintain daily communication with Admissions Department and Team Directors. Work with Hospice Care Consultants to develop and implement individualized marketing plans. Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups. Act as a liaison with AccentCare Hospice Medical Director, Team Physicians and Hospice Physicians on behalf of providers and patients/families. Make inpatient visits at contracted facilities and communicate with Admissions Department, Team Directors and/or Primary Nurse. Provide consultation on hospice and palliative care.
