Work Environment

This position is based onsite at our assisted living facility in Binghamton, NY, where you will be part of a collaborative care team that includes physicians, other Nurse Practitioners or Physician Assistants, medical assistants, and registered nurses. Key Responsibilities

• Deliver exceptional primary care to a diverse patient population, addressing both routine and acute healthcare needs.
• Conduct comprehensive office visits and annual wellness assessments, focusing on patient health holistically and ensuring accurate, thorough documentation.
• Coordinate care effectively with various healthcare providers, specialists, and diagnostic testing facilities, ensuring seamless patient experiences.
• Lead population health initiatives to promote preventive care measures, such as regular screenings, and monitor compliance with evidence-based guidelines.
• Assist the care team with patient triage, providing timely follow-ups and outreach as necessary.
• Educate patients about their health conditions, treatment options, and care plans to promote informed, engaged patient participation in their care.
• Engage in promotional and outreach activities to enhance community awareness of our services.
• Conduct home visits when necessary, ensuring continuity of care and addressing patient needs outside of the facility.
• Report directly to the Medical Director and collaborate closely with operational leadership and other healthcare professionals.

Patient Population / Client Focus

Our primary focus is on older adults who receive Medicare. The Nurse Practitioner will play a crucial role in ensuring that our clients receive high-quality, personalized healthcare experiences that respect their individual needs and promote their overall well-being. By emphasizing preventive care and chronic disease management, you will help empower our patients to live their lives to the fullest in their community settings.

Qualifications

• Master's Degree in Nursing, Doctor of Nursing Practice, or graduate from an accredited Nurse Practitioner Certificate Program.
• Nationally recognized certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• A valid, active Nurse Practitioner license in the state of New York, free of probationary status.
• An active DEA license, enabling you to prescribe medications responsibly.
• Proven experience in primary care settings focusing on internal medicine, geriatrics, or family practice.
• Familiarity with HCC documentation and ICD-10 coding is essential for accurate patient records.
• A genuine passion for teamwork and cross-functional collaboration.

Preferred Experience

• Experience working in assisted living or skilled nursing environments is highly desirable.
• Proficiency in utilizing electronic health record systems and patient management software to streamline workflows and documentation.
• Background in geriatrics or chronic disease management provides an advantage in understanding patient needs.

Compensation and Benefits

We offer a competitive salary in the range of USD 120,000 to 160,000 per year, commensurate with experience.
