Key Responsibilities

• Provide high-quality, patient-centered care in accordance with established national guidelines and community standards.
• Conduct comprehensive evaluations of patients, analyzing clinical data to assess their medical needs and develop tailored treatment plans.
• Collaborate with office staff to enhance operational efficiency, ensuring smooth workflow and effective communication within the team.
• Engage with practice management and hospital administration to represent the practice effectively, aligning operational goals with patient care objectives.
• Develop strong relationships with patients, actively promoting their health and wellbeing through education and support.
• Maintain regulatory compliance in all aspects of patient care and office practices.
• Document patient care activities in a timely and accurate manner, ensuring that all medical records meet requirements for billing, regulatory standards, and continuity of care.
• Continually assess and adapt care practices to meet the ever-evolving regulatory and clinical standards.

Patient Population / Client Focus

You will provide care primarily to adult and geriatric patients within the outpatient setting, addressing a range of acute and chronic health conditions. The aim is to promote health and wellness, manage chronic diseases, and provide preventive care tailored to individual patient needs. Building trust and rapport with patients will be central to your role, empowering them through education and support on lifestyle changes and treatment options.

Qualifications

• Active New York State certification and registration as a Family, Adult, or Pediatric Nurse Practitioner.
• Board certification from the American Association of Nurse Practitioners (AANP).
• Current DEA license.
• Ability to obtain privileges from F. F. Thompson Health.
• Knowledge of ICD10 coding is preferred but not mandatory.

Preferred Experience

• Prior experience in an outpatient clinical setting as a Nurse Practitioner is strongly preferred, providing a background in managing diverse patient populations.

Compensation and Benefits
