Work Environment

This role is entirely onsite, requiring travel to various patient homes within the Binghamton area. Key Responsibilities

• Perform comprehensive health assessments for patients of all ages, including physical examinations, medical histories, and development of individual care plans tailored to specific health needs.
• Order and interpret diagnostic tests such as lab work, imaging, and other assessments to accurately diagnose medical conditions.
• Collaborate with a multidisciplinary care team to formulate and implement evidence-based, patient-centered care plans that address both immediate and long-term health goals.
• Prescribe medications and develop treatment regimens, ensuring adherence to medical standards and best practices.
• Provide crucial health education and counseling to patients and their families, empowering them with knowledge about their conditions and self-care strategies.
• Maintain meticulous documentation of patient interactions, treatment plans, and progress notes within our electronic health record (EHR) system to ensure continuity of care.
• Participate actively in team meetings and case conferences, sharing insights on patient care and collaborating on clinical decisions.
• Engage in continuous quality improvement initiatives aimed at optimizing patient outcomes and enhancing the efficiency of healthcare delivery.

Patient Population / Client Focus

This role encompasses a wide range of patients, from newborns to the elderly, with varying health needs including acute and chronic conditions. The Family Nurse Practitioner will focus on preventive care, emphasizing screenings and wellness consultations, as well as managing patients with chronic illnesses such as diabetes and hypertension. Qualifications

• A valid and unrestricted Nurse Practitioner license in the state of New York is mandatory.
