This full-time position will operate onsite in our outpatient office, providing comprehensive care to patients with cardiac conditions. As an NP, you will be pivotal in diagnosing and managing both chronic and acute cardiological issues, ensuring patients receive optimal treatment and education.

Key Responsibilities

• Conduct thorough assessments by obtaining patient histories and performing both comprehensive and targeted physical examinations.
• Order and interpret diagnostic tests, applying clinical expertise to diagnose and manage patient conditions effectively.
• Formulate individualized, goal-directed care plans that encompass health maintenance and appropriate nursing interventions, in alignment with established protocols.
• Collaborate with the medical team to review and adjust treatment plans as necessary, ensuring continuity and quality of care.
• Educate patients and their caregivers on cardiac health, disease management, and self-care strategies to optimize health outcomes.
• Coordinate hospital admissions and collaborate with inpatient teams for seamless discharges, ensuring safe transitions in care.
• Participate in multidisciplinary team meetings, effectively communicating with social workers, nutritionists, and pharmacists to synthesize comprehensive care plans.
• Document all patient interactions meticulously in the electronic medical records (EMR) system to uphold standards of care and maintain detailed patient histories.
• Actively engage in research and clinical protocols, leveraging findings to enhance practice and inform patient management strategies.
• Work diligently to maintain a therapeutic and consumer-oriented environment, ensuring safety and satisfaction among patients.

Patient Population / Client Focus

The NP will primarily focus on patients with diverse cardiovascular conditions, including advanced heart failure, pulmonary arterial hypertension, and individuals undergoing heart transplants. You will address the complex needs of these patients, providing crucial support for those requiring advanced therapies such as intravenous medication therapy and mechanical circulatory support.

Qualifications

• A Master’s degree in Nursing, with a specialization as a Nurse Practitioner, particularly in adult care.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Board certification as a Nurse Practitioner.
• Valid BCLS, ACLS, DEA, and NPI certifications.
• Proficiency in using electronic medical records systems, as well as competency in MS Office applications like Word, PowerPoint, and Excel.

Preferred Experience

• Two years of experience working in a hospital or outpatient setting, preferably within cardiology.
• Familiarity with management and treatment of advanced heart failure patients, and knowledge of current cardiology practices.
• Experience in an outpatient care environment offering advanced therapies.

Compensation and Benefits

The Nurse Practitioner position at Gotham Enterprises Ltd offers a competitive salary range of USD 120,000 – 220,000 annually, commensurate with experience and qualifications.
