Full-Time Nurse Practitioner in Internal Medicine - Outpatient Care

Work Environment

The successful Nurse Practitioner will work onsite at our outpatient practice located in Binghamton, NY. Your schedule will be conducive to work-life balance, with office hours from Monday to Friday, 8:00 AM to 5:00 PM. Key Responsibilities

• Deliver exceptional primary care tailored to individual patient needs.
• Conduct thorough office visits addressing both routine and acute medical issues.
• Manage annual wellness visits and health risk assessments with comprehensive documentation.
• Coordinate care with a variety of healthcare providers, specialists, and testing facilities to ensure seamless patient experiences.
• Lead initiatives aimed at population health, such as ensuring regular cancer screenings for eligible patients.
• Engage in phone triage and outreach to support the care team and patients.
• Educate patients about their health conditions, treatment plans, and lifestyle modifications.
• Participate in community outreach and promotional activities to enhance patient engagement.
• Conduct home visits when necessary to provide care in the community.
• Report directly to the Medical Director and collaborate closely with operational leadership to align practices with organizational goals.

Patient Population / Client Focus

The Nurse Practitioner will primarily focus on serving adults on Medicare, particularly older adults who require specialized attention to maintain their health and well-being. Our model emphasizes personalized care that seeks to understand each patient's unique circumstances, wishes, and concerns, ensuring they remain engaged and informed about their healthcare plans.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Certificate Program.
• National certification as a Family Nurse Practitioner, Adult Nurse Practitioner, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Possession of an active and non-probationary Nurse Practitioner license in New York State.
• Current DEA license is required.
• Strong background in primary care, preferably with experience in internal medicine, geriatrics, or family medicine.

Preferred Experience

• Familiarity with Hierarchical Condition Categories (HCC) documentation and ICD-10 coding.
• Experience conducting Health Risk Assessments (HRAs) to inform care planning.
• A collaborative mindset with a willingness to engage in cross-functional teamwork.
• Bilingual proficiency relevant to our patient population is an asset.

Compensation and Benefits

We offer a competitive salary ranging from USD 115,000 to 150,000 annually, adjusted based on experience and expertise. In addition to a lucrative salary, our Nurse Practitioners receive:
