Work Environment

This position is performed onsite in a collaborative environment within a school, providing care to adolescent patients and helping them understand and manage their cardiovascular health. Key Responsibilities

• Conduct thorough patient assessments, gathering detailed medical histories, performing physical examinations, and executing diagnostic tests relevant to cardiovascular health.
• Develop and implement tailored treatment plans in collaboration with cardiologists and interdisciplinary healthcare teams, adjusting strategies based on ongoing patient evaluations.
• Manage chronic cardiovascular conditions, monitoring symptoms and treatment efficacy for patients with hypertension, heart failure, and arrhythmias.
• Provide vital education to patients and families about lifestyle modifications, medication management, and preventive strategies to enhance cardiovascular health outcomes.
• Perform and analyze diagnostic procedures such as electrocardiograms (EKGs), stress tests, and echocardiograms, ensuring accurate interpretations to guide treatment decisions.
• Prescribe medications according to established clinical protocols, ensuring compliance with best practices and patient safety guidelines.
• Engage in routine follow-up care, conducting regular check-ups and adjusting treatment plans as necessary to optimize health results.
• Collaborate actively with multidisciplinary healthcare teams, including nutritionists, physical therapists, and mental health professionals, to provide comprehensive care.
• Maintain precise, up-to-date medical documentation and patient records to comply with regulatory requirements and facilitate care continuity.

This role focuses on a unique population, primarily serving school-age children and adolescents who may be at risk for or diagnosed with cardiovascular conditions. You will play a crucial role in guiding young patients and their families toward healthier lifestyles and empowering them to take an active role in their heart health management.

Qualifications

• Master's degree in Nursing from an accredited program, with specialization in Family or Adult NP preferred.
• Current and valid Nurse Practitioner license in the state of New York.
• Board certification as a Nurse Practitioner in Cardiology is highly desirable.
• Minimum of two years of experience in cardiology or related healthcare field.
• Exceptional clinical skills in performing and interpreting cardiovascular diagnostic tests.
• Strong communication and interpersonal abilities, essential for patient interaction and collaboration with healthcare teams.

Compensation and Benefits
