Work Environment

The role is based onsite within a vibrant school environment, providing a unique opportunity to engage with children and families in their everyday settings. You will work collaboratively with educators, administrative staff, and other healthcare professionals to ensure a supportive and responsive healthcare environment. Your daily schedule will typically be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, you may participate in community outreach events and health fairs on weekends, contributing to our mission beyond the school walls.

Key Responsibilities

• Conduct comprehensive health assessments and screenings, identifying both acute and chronic health issues among students and their families.
• Diagnose and manage a broad range of medical conditions while delivering preventive care tailored to individual needs.
• Collaborate closely with community organizations to build and implement health education initiatives that empower families through knowledge about nutrition, exercise, and preventative health.
• Foster a culturally sensitive care practice, advocating for vulnerable populations and ensuring equitable access to healthcare resources.
• Coordinate referrals and follow-up care seamlessly to maintain continuity for patients, advocating for their needs within the healthcare system.
• Maintain detailed and accurate medical documentation in compliance with all applicable regulations and standards, ensuring that all patient interactions are recorded diligently.
• Actively participate in community outreach events and health fairs to promote service awareness and provide essential health screenings.
• Stay informed about community health trends, evidence-based treatments, and policy changes that may affect patient care delivery.

Qualifications

• Current Nurse Practitioner certification.
• Active nursing license in the state of New York.
• Master's degree in Nursing from an accredited institution.
• A minimum of 3 years’ experience as a Nurse Practitioner, particularly in community health or public health settings.
• Exceptional interpersonal and communication skills, capable of building rapport with diverse populations.

Compensation and Benefits
