Position Summary


The RN Case Manager coordinates and manages nursing services for individuals with Intellectual and Developmental Disabilities (IDD) by conducting comprehensive nursing assessments, developing and monitoring individualized care plans, coordinating interdisciplinary services, and promoting quality, person-centered care. This position serves as a clinical resource for individuals, families, caregivers, and healthcare providers while ensuring compliance with Ohio nursing regulations, Medicaid requirements, and agency policies.


Position Scope


The RN Case Manager provides clinical oversight and case management for individuals receiving home and community-based services. The position is responsible for nursing assessments, care coordination, staff support, nursing delegation oversight, regulatory compliance, quality assurance, and collaboration with interdisciplinary teams to ensure safe, effective, and individualized care.


Key Responsibilities


The RN Case Manager is responsible for coordinating nursing care, conducting assessments and reassessments, developing and monitoring individualized care plans, overseeing nursing delegation, documentation, and regulatory compliance; supporting staffing and onboarding activities; supervising and educating field nursing staff, collaborating with individuals, families, physicians, Service Support Administrators, and interdisciplinary teams, promoting quality improvement initiatives, and ensuring the delivery of safe, person-centered services.

Confidentiality


The RN Case Manager is responsible for maintaining the confidentiality of participant health information, employee records, clinical documentation, investigations, and proprietary agency information in accordance with HIPAA, Ohio privacy laws, and agency policies.


Documentation Requirements


The RN Case Manager is responsible for maintaining:

• Nursing assessments and reassessments.
• Individualized care plans.
• Nursing progress notes.
• Incident reports.
• Medication-related documentation.
• Clinical supervision records.
• Staff competency documentation.
• Quality assurance reports.
• Records required by DODD, Medicaid, and agency policies.

Certifications


Current, unrestricted Ohio or Multistate Registered Nurse (RN) license, CPR and First Aid certification, and completion of all agency-required clinical, compliance, and regulatory training. Additional certifications in case management or developmental disabilities are considered an asset.


Education


Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited nursing program. A BSN is preferred.


Experience


Minimum of two (2) years of nursing experience, preferably in home health, case management, developmental disabilities, community health, or care coordination. Experience working with individuals with Intellectual and Developmental Disabilities and interdisciplinary healthcare teams is preferred.


Technical Skills


Knowledge of nursing assessment and case management principles, proficiency with electronic health records (EHR) and clinical documentation systems, Microsoft Office applications, care planning software, and the ability to analyze clinical information, maintain accurate documentation, and coordinate multidisciplinary services.


Languages


Ability to read, write, and communicate effectively in English. Bilingual proficiency is considered an asset based on participant needs.


Driver License


Valid Ohio driver's license, reliable transportation, an acceptable driving record, and the ability to meet agency insurance requirements.


Travel Requirement


Frequent local travel is required to participant homes, community locations, healthcare facilities, and agency offices throughout the assigned service area. Occasional travel for training, meetings, and professional development may also be required.
