As an employer, we seek to provide an environment of growth and development for our employees that is consistent with recognizing the rights, needs, and wants of all people.Purpose:To provide support and assistance to individuals with intellectual and developmental disabilities to enable them to live valued lives in the community.Qualifications:* Must successfully complete the required pre-employment Background and Drug Screenings* Must possess a valid Driver's License with all necessary endorsements and have a minimum of 3 years of driving experience.* Must possess advanced computer skills* Must possess a High School Diploma or equivalent, Bachelor’s Degree preferred* Experience working in the field of intellectual/developmental disabilities preferred* Minimum one year of experience in writing program plans, case notes or other relevant documentation requiredEssential Functions:* Responsible for the development, implementation and coordination of individual’s program plans* Develop, implement and monitor individualized training programs to meet identified priority training needs* Ensures all plans are person centered and reflect the individual’s hopes and dreams through active participation wherever possible* Ensure all documentation is accurate and complete according to regulatory requirements* Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed* Performs other duties as assignedThis position will remain open until filled.
