Registered Nurse – Case Manager

Schedule: Day 5x8-Hour (08:00 - 16:30)

Assignment Details:

Floating Expectation: No

Ratios: 1:20

RTO Restrictions: No more than 7 days

Requirements

• California state license required
• Minimum 2 years of acute care hospital case management experience in care coordination and discharge planning
• Pendant license accepted but must be licensed upon start date
• Weekend rotation required to meet department needs

Responsibilities

• Conduct assessments and develop care plans for patients
• Coordinate and facilitate discharge planning processes
• Collaborate with multidisciplinary teams to optimize patient outcomes
• Monitor patient progress and adjust care plans as necessary
• Provide education and support to patients and families
