What You'll Do

As the Social Services Coordinator, you will:

• Build meaningful relationships with residents and their families.
• Assess residents' social, wellness, and support needs.
• Coordinate transitions throughout our continuum of care, including Independent Living, Assisted Living, Memory Care, Skilled Nursing, and Rehabilitation.
• Connect residents with internal and external community resources.
• Advocate for residents while serving as a liaison between residents, families, physicians, and community departments.
• Monitor resident wellness and proactively identify opportunities for additional support.
• Coordinate health education programs, support groups, and wellness initiatives.
• Participate in interdisciplinary meetings to promote resident-centered care.
• Help create a welcoming experience for new residents through orientation and ongoing support.
• Promote resident engagement and overall well-being through collaboration and compassionate service.

Qualifications

• Bachelor's degree in Social Work or a related field required.
• Experience in senior living, healthcare, case management, social services, or care coordination preferred.
• Strong interpersonal, organizational, and communication skills.
• Ability to maintain confidentiality and exercise sound judgment.
• Ability to manage multiple priorities in a fast-paced environment.
