Your Responsibilities

• Conducting thorough assessments of patients, ordering necessary diagnostic tests, and developing customized care plans
• Diagnosing and treating acute, episodic, or chronic health issues in partnership with physicians or autonomously (as per credential eligibility)
• Educating patients and their families on prevention, wellness, and disease management strategies
• Prescribing medications and therapies within your practice scope
• Engaging in patient follow-up and managing chronic care
• Collaborating with interdisciplinary teams to ensure cohesive, comprehensive care
• Accurately documenting within electronic medical records (EMR)

Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Graduation from an accredited Nurse Practitioner program

Essential Certifications

• Current Nurse Practitioner licensure in New York State
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or a specialty equivalent
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to obtain these within 90 days)
