Post-Acute Care Physician Opening Wilmington, DEFull TimePermanent PositionSeeking BC/BE Physician (IM/FM/EM or related) to join post-acute care team in Wilmington, DelawarePosition Highlights:Skilled nursing facility (SNF) and assisted living facility (ALF) settingsFlexible rounding schedule with strong work/life balanceFocus on post-acute and long-term patient managementCollaborative interdisciplinary team environmentLeadership opportunities including oversight and mentorship of nurse practitionersInvolvement in care coordination and quality improvement initiativesResponsibilities:Provide primary care rounding in SNF and ALF settingsManage chronic conditions and acute medical issuesDevelop and implement patient care plansCollaborate with clinical teams and facility leadershipMentor and supervise nurse practitionersParticipate in clinical case reviews and care coordination effortsQualifications:Board Eligible or Board Certified in Internal Medicine, Family Medicine, Emergency Medicine, or related fieldActive Delaware license or ability to obtainActive DEA or ability to obtainCompensation & Benefits:Salary range $210,579 to $271,579 annuallyUncapped productivity bonus potentialFlexible schedulingComprehensive benefits packagePaid professional liability insurance with tail coverageContinuing medical education (CME) and professional development support
