Job Title Physical Therapist (Travel) – Fountain, Colorado Contract Details 6‑month outpatient contract starting August 17th. Schedule & Compensation Start Date: ASAP Duration: 13 weeks Hours: 36 per week (12-hour shifts) Shift Pattern: 5×8.0 or 12‑hour flexible Employment Type: Travel Compensation: Pay package based on 12‑hour shifts, tax‑free stipend determined Responsibilities Conduct comprehensive evaluations of assigned patients. Utilize clinical reasoning to develop treatment plans and goals focused on patient discharge. Complete accurate and timely medical record documentation, including goal writing and quality assessments. Ensure services are provided under valid physician orders and within insurance approvals. Participate in student programs, providing a positive learning environment. Develop an individualized professional growth plan through continuing education and professional activities. Adhere to discipline‑specific State Practice Act and maintain professional licensure in good standing. Understand and contribute to organizational performance improvement initiatives. Qualifications & Experience Degree in Physical Therapy from an accredited school. 6 months to 1 year of outpatient clinical experience. CPR Certification.
