Key Responsibilities

• Conduct comprehensive evaluations of patients to determine their physical capabilities, restrictions, and rehabilitation needs based on medical histories and physician referrals.
• Establish clear therapeutic goals and develop individualized treatment plans that address each patient's unique conditions and functional limitations.
• Utilize evidence-based techniques and interventions to deliver therapy effectively, monitoring patient progress and adapting treatment plans as necessary.
• Maintain accurate and timely documentation of patient evaluations, treatment sessions, and discharge summaries, ensuring compliance with healthcare regulations and reimbursement protocols.
• Supervise and guide the activities of Physical Therapist Assistants (PTAs) and support staff, promoting a team-oriented approach to patient care and education.
• Engage in interdisciplinary collaboration with physicians, nurses, occupational therapists, and speech-language pathologists to coordinate comprehensive care.
• Participate in staff meetings and continuous education programs to stay current on best practices and innovations in physical therapy.
• Actively contribute to maintaining a positive atmosphere that encourages professional growth and high standards of care.

Patient Population / Client Focus

As a Physical Therapist at Guided Grace, you will provide care to diverse patient demographics, including individuals recovering from surgery, sports injuries, neurological conditions, and chronic pain. Qualifications

• Associate's Degree from an accredited institution in Physical Therapy, with a Bachelor's Degree preferred.
• Active Physical Therapist license issued by the state of Virginia.
• Strong verbal and written communication skills for effective patient interaction and collaboration with healthcare team members.
• Proficiency with electronic health record (EHR) systems and Microsoft Office Suite for documentation and reporting.

Preferred Experience

• Minimum of two years of experience in a clinical physical therapy setting, preferably in outpatient rehabilitation.
• Experience working with a variety of patient populations and knowledge of diverse therapeutic modalities.
• Demonstrated ability to work well in a multidisciplinary team and possess strong leadership skills.

Compensation and Benefits
