Assess patient and family needs, facilitate discharge planning, and coordinate necessary referrals and services. Develop, monitor, and update individualized care plans with clearly defined goals and expected outcomes. Collaborate with physicians, nurses, social workers, and ancillary staff to ensure safe transitions, appropriate resource use, and timely documentation using the electronic medical record. Conduct ongoing risk assessments, identify barriers to care, and implement interventions to optimize patient outcomes and utilization. Ensure documentation accuracy, compliance with regulatory standards, and adherence to evidence-based practices in case management.
