Physical Therapist - Sports Medicine The THSM Rehab Clinic department is looking for a top skilled PT. Work location: Texas Health Sports Medicine, 3920 W Wheatland Rd Suite 124, Dallas, TX 75237. Work hours: Full Time (40 Hours) Day Shift. Department Highlights Team based environment Workplace culture to the nth degree Great benefits / 401k / PTO We operate on lean principles and rely heavily on a team atmosphere and individual performance Highly engaged management Education Bachelor’s Degree in Physical Therapy upon hire required Master’s Degree in Physical Therapy upon hire required Doctorate in Physical Therapy upon hire required BLS Certification Basic Cardiac Life Support (BCLS) prior to providing independent patient care and maintained quarterly required Experience 2 years prior experience preferred Licenses and Certifications PT - Physical Therapist Texas PT License upon hire required BCLS - Basic Cardiac Life Support prior to providing independent patient care and maintained quarterly required Other Board‑certified Clinical Specialist upon hire preferred What You Will Do Perform age‑appropriate patient assessment and establish plan of care Obtain/verify physician order for therapy and validate for appropriateness Review available medical records and obtain pertinent medical history from patient or family Ensure that patient has given informed consent to treatment Evaluate each patient using age and diagnosis appropriate assessment tests and measurements, including appropriate outcome assessment tool(s) Integrate assessment results to identify functional problems and prioritize care needs with patient and family Set appropriate, measurable, functional goals with time frames to address the identified problems, including patient/family goals and preferences Determine plan for care and/or appropriate therapeutic interventions to achieve stated goals Initiate therapy discharge planning in collaboration with care team and patient based on prognosis and expected discharge disposition Demonstrate competence in the performance of age‑appropriate patient assessment and individualize care based on age‑specific needs Perform age‑appropriate patient treatment using therapeutic techniques appropriate to patient age and desire for treatment Perform treatment procedures that are within the scope of the professional license Perform and delegate technical procedures appropriately and safely Utilize a variety of treatment techniques to achieve goals, if necessary Determine patient response to treatment and evaluate progress (reassessment) Educate patient and family through instruction, demonstration, and/or appropriate handouts in treatment plan, safety issues, home programs/self‑management and assistive equipment Maintain patient safety throughout treatment Communicate with physicians and other healthcare personnel as appropriate regarding patient care issues Motivate non‑compliant or unmotivated patients to participate in therapy Demonstrate professional accountability Utilize available resources (equipment, support personnel, educational resources) to maximize efficiency and productivity and to meet personal developmental needs Participate in orientation and training of less experienced staff, students, volunteers and observers Participate in cross‑training to treat any diagnosis or level of care as required Attend and/or lead hospital, department, and community meetings/educational in‑services Meet established productivity targets Represent department in a positive manner to hospital/system/community Complete assigned work within scheduled hours and offer assistance as required Seek assistance, supervision, or consultation as needed and report to supervisor any problems and/or solutions regarding delivery of patient care Participate in continuous improvement activities and/or teams Demonstrate commitment to the maintenance of a clean, safe work environment that is compliant with departmental, hospital, and regulatory agency requirements Work in partnership with peers, other disciplines, and departmental personnel Participate in departmental safety drills/emergency procedures as required Perform appropriate documentation: document all aspects of assessment, plan of care, and re‑certifications in accordance with departmental, TJC, Medicare, and other applicable regulatory guidelines Document all aspects of each treatment session in accordance with departmental, TJC, Medicare, and other applicable regulatory guidelines Obtain physician signature on all care plans and re‑certifications, providing proof of attempts to obtain signature, such as documenting contacts in medical record and retaining fax cover letters Accurately submit patient charges and records procedures according to departmental policy Document assessment of education needs, education provided, and patient understanding of education provided in accordance with departmental, TJC, Medicare, and other applicable regulatory guidelines Complete discharge summary for each patient in accordance with departmental, TJC, Medicare, and other applicable regulatory guidelines Complete all documentation within established time frame;
