Traveling RN Case Manager - Home Health & Telemedicine
Your Health Organization
Your Health Organization is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide in-home patient care across the Savannah area. The Nurse Case Manager will travel to patients\' homes, long-term care facilities, and other sites to evaluate needs and initiate telemedicine visits.
Provide quality healthcare in accordance with applicable laws and policies. The role requires travel to patients\' homes daily, valid RN/LPN license, and strong communication skills.
