Case Management Associate - 20 hours per week

Under the supervision of the unit supervisor, the Case Management Associate performs supportive and adjunctive case management and community support services. This position provides services to adults experiencing serious and persistent mental illness within the Carver County Community Support Program (CSP). This position may be safety-sensitive, depending on assigned duties and responsibilities. Responsibilities Perform outreach to potential clients, including assistance in completing forms and understanding the nature of available services and how potential clients may qualify for and benefit from these services. Assist the case manager in work with clients on a variety of housing supports, including tenancy rights, resolving conflicts with neighbors and/or landlords, and helping clients keep current with rent payments. Lead a variety of supportive and rehabilitative groups to adults with serious and persistent mental illness, as directed by case managers or the case management supervisor. Provide 1:1 support to assist adults in accessing services and resources, including provision of transportation. Other duties assigned. Qualifications High school diploma or GED. Valid MN driver's license or evidence of equivalent mobility, depending on program or assignment. Preferred Qualifications One year experience working directly with clients in a human services/community services area and setting. All applications will be screened for minimum qualifications, scored and ranked. The highest ranked candidates meeting minimum qualifications will be recommended for the scored oral examinations (interviews).
