Every role here contributes to that mission - and as a Care Coordinator I, you'll be a dedicated point of contact for patients and caregivers, helping them feel informed, supported, and confident throughout their treatment journey.We are seeking a compassionate, detail-oriented Care Coordinator I to support patients and caregivers through the practical aspects of treatment. In this patient-facing role, you will help coordinate device logistics, provide travel and technical support, place supply and equipment orders, and ensure timely, accurate documentation across company systems. This is a full-time, non-exempt position reporting to the Manager, Care Center. The role may be fully remote for candidates who are not local to New Hampshire, or hybrid for candidates who live near our Portsmouth, NH office. Remote team members must be able to travel to Portsmouth or other company locations for initial onboarding and training, as well as occasional team events or additional training.Serve as a dedicated contact for patients and caregivers, providing responsive support by phone and video. Complete inbound and outbound calls to help patients navigate treatment-related needs and next steps. Explain device logistics and provide travel and technical support throughout the patient treatment journey. Place supply and equipment orders and help ensure patients have the materials they need to begin and remain on treatment. Create a supportive experience that helps patients and caregivers feel valued, secure, and confident. Maintain accurate data entry and documentation in company-specific CRM and ERP platforms in accordance with standard operating procedures. Collaborate closely with Care Center leadership and team members to address patient needs and operational priorities. Provide occasional rotating on-call coverage outside normal business hours, including weekends and holidays, as needed. Participate in scheduled team meetings, company events, onboarding, and training activities.You are a thoughtful communicator who enjoys helping people and can balance empathy with accuracy and follow-through. You are comfortable speaking directly with patients and caregivers, handling multiple requests, documenting information carefully, and working within established processes in a regulated healthcare environment. You bring a collaborative mindset, strong organizational skills, and a patient-forward approach to every interaction. You are comfortable learning new systems, asking questions when needed, and helping create a reliable, reassuring experience for patients and their caregivers.Minimum Qualifications:2-year college degree preferredPatient-facing healthcare experience preferredWorking knowledge of Microsoft Excel, Word, and OutlookUnderstanding of Protected Health Information (PHI) and HIPAA Privacy and Security guidelinesAbility to communicate clearly and professionally with patients, caregivers, and internal teamsAbility to move equipment weighing up to 50 poundsPreferred Qualifications:Basic knowledge of Medicare, managed care, third-party payers, and MedicaidFamiliarity with the legal and regulatory environment applicable to healthcare and medical technology organizations, including relevant industry guidelinesAbility to fluently speak and understand SpanishReport to the Manager, Care Center and work closely with Care Center leadership and teammates. Work remotely if not local to New Hampshire, or in a hybrid model if located near the Portsmouth, NH office. Travel to Portsmouth, NH or other company locations for initial onboarding and training and approximately 1–2 times per year for additional training and/or team events.
