Key Responsibilities
- Provide physical therapy evaluations and treatments to patients in a home health setting
- Complete and maintain accurate OASIS documentation in accordance with home health requirements
- Develop and implement individualized patient care plans based on assessment findings
- Monitor and document patient progress, adjusting treatment plans as appropriate
- Collaborate with interdisciplinary care team to ensure coordinated patient outcomes
Required Qualifications
- Active Physical Therapist license valid in Virginia (in hand at time of submittal or proof of compact license)
- Minimum 1 year of experience in a home health setting
- Proficiency in OASIS documentation
- BLS certification through the American Heart Association (AHA)
- One supervisory reference required at time of submittal
Preferred Experience / Skills
- Experience with home health-specific electronic medical record systems
- Strong communication and patient education skills in a home-based care environment
