Registered Nurse (RN) - Home Health Full-Time | 40 Hours per Week Position Summary We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The RN will provide skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care while promoting independence, safety, and optimal health outcomes. Essential Responsibilities Conduct comprehensive patient assessments and develop individualized care plans. Provide skilled nursing services in accordance with physician orders and agency policies. Administer medications, treatments, and wound care as prescribed. Monitor patient condition and document changes in health status. Educate patients, families, and caregivers regarding disease management, medications, and home care needs. Coordinate care with physicians, therapists, social workers, and other interdisciplinary team members. Maintain accurate and timely clinical documentation in the electronic medical record. Assess patient safety needs and recommend appropriate interventions and resources. Participate in case conferences and care coordination meetings. Ensure compliance with all federal, state, and agency regulations and standards. Respond appropriately to patient concerns and provide exceptional customer service. Qualifications Active Registered Nurse (RN) license in the state of practice. Graduate of an accredited nursing program. Current BLS/CPR certification. Home Health experience preferred but not required. Strong assessment, communication, and organizational skills. Ability to work independently and manage a flexible schedule. Valid driver's license, reliable transportation, and proof of auto insurance.
