Bilingual proficiency in English and another language. Previous experience in a community health setting. Responsibilities: Conduct thorough clinical assessments to evaluate patients' health status and needs. Develop and implement individualized treatment plans in collaboration with patients and healthcare teams. Provide family and patient counseling to support mental and emotional well-being. Administer behavioral health services, including therapy and medication management. Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations. Skills: The required skills of family counseling and patient counseling will be utilized daily as you engage with patients to understand their concerns and provide emotional support.
