We're Hiring: Travel Registered Nurse (RN) – Home Health (Days) | Littleton, NH

Location: Littleton, New Hampshire

Shift: Days | Monday – Friday | 8:00 AM – 5:00 PM

Schedule: 40 Hours/Week (5x8s)

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care for Home Health and Hospice patients.
• Provide skilled nursing services, including disease management, wound care, medication administration, injections, and IV therapy.
• Monitor patient progress, evaluate treatment outcomes, and update care plans as needed.
• Educate patients and family members regarding diagnoses, medications, treatments, and self-care management.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care.
• Accurately document patient assessments, interventions, and outcomes in electronic medical records (EMR).
• Deliver compassionate end-of-life and palliative care support when applicable.
• Maintain compliance with agency policies, nursing standards, and regulatory requirements.
• Demonstrate flexibility and adaptability in response to changing patient needs and schedules.
• Travel to patient homes within the assigned territory while maintaining professional and patient-centered care.

Requirements

✔ Active New Hampshire RN License or Compact License
✔ Active BLS Certification (AHA)
✔ Minimum 18 months of recent Home Health experience (Required)
✔ Previous Hospice experience (Preferred)
✔ 2–5 years of Medical-Surgical experience (Preferred)
✔ Ability to work independently with minimal supervision
✔ Strong assessment, critical thinking, and communication skills
✔ Computer proficiency and experience with EMR documentation
✔ Valid Driver's License and Current Auto Insurance
✔ Ability to provide care in diverse home environments and patient populations
