Work Environment

This full-time position requires working onsite in a home care setting, where you will conduct evaluations and provide treatment in the patients' homes. Flexibility is essential, as this role involves variable weekend shifts alongside a standard 40-hour workweek. Key Responsibilities

• Conduct thorough examinations of patients with functional limitations, evaluating their individual needs to develop effective, personalized treatment plans.
• Provide expert consultation to healthcare providers regarding each patient's continuum of care, especially during transitions in treatment.
• Deliver compassionate, culturally-sensitive interactions with all patients, adhering to our established customer service standards.
• Engage in continual assessment of treatment methods, incorporating patient feedback and clinical findings to adjust care strategies as needed.
• Document all evaluations, treatment plans, and outcomes accurately, ensuring compliance with all relevant laws, regulations, and professional standards.
• Maintain active and open communication with interdisciplinary teams, including physicians and referral sources, to optimize patient care processes.
• Participate in community outreach, quality improvement initiatives, and marketing efforts to promote physical therapy services and uphold cost containment.
• Attend and contribute to staff meetings, maintaining professional growth through continuing education and self-directed learning.
• Supervise and mentor assigned support staff, fostering a collaborative learning environment.
• Manage departmental resources wisely, ensuring efficient use of materials and time in delivering care.

Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• A valid state license to practice Physical Therapy in New York is essential.

• Proficiency in word processing and computer skills is required for documentation and reporting.

Compensation and Benefits
