Registered Nurse (RN) - Case Management

Registered Nurse – Home Health

Schedule: Shift: 7:00 AM - 7:00 PM

Assignment Details:

• Guaranteed Hours: 36.00
• Contract Weeks: 13

Requirements:

• Bachelor's Degree in Nursing (BSN) preferred.
• Registered Nurse (RN) licensure in the state of practice required.
• Current Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor required.
• Valid Driver's License and must be at least 21 years of age or older required.
• Current automobile insurance and a reliable automobile required.
• At least two years of recent experience in the field preferred.

Roles & Responsibilities:

• Plan, organize, and direct home care services to meet patient needs.
• Manage a patient caseload effectively and efficiently.
• Provide independent patient care and collaborate with other core disciplines to meet care goals.
• Conduct comprehensive assessments of patients' needs and implement care plans accordingly.
• Document patient care delivery, status, and response to treatment in a timely manner.
• Educate patients and families on healthcare needs and nursing care.
