Manage a caseload of patients, conducting thorough assessments and developing individualized care plans. Provide skilled nursing interventions to address physical, emotional, and psychosocial needs. Serve as the primary point of contact for patients, families, and the interdisciplinary hospice team to ensure seamless communication and continuity of care. Handle symptom management, medication administration, patient and family education, and coordination with physicians and support staff. Monitor and evaluate care effectiveness, document care plans and progress, and adjust as needed to meet patient goals.
