Responsibilities
- Coordinate and deliver comprehensive end-of-life care to terminally ill patients in line with hospice philosophy and regulatory guidelines
- Manage a caseload of patients, performing thorough assessments and developing individualized care plans
- Provide skilled nursing interventions to address physical, emotional, and psychosocial needs
- Serve as the primary point of contact for patients, families, and the interdisciplinary care team
- Ensure effective care communication and continuity across physicians, nursing, and support staff
- Manage symptom control, medication administration, patient and family education, and ongoing evaluation of care effectiveness

Requirements
- Active Registered Nurse license in California
- Current BLS certification
- Experience in hospice, palliative, or case management nursing preferred
- Education: Associate's degree in Nursing (ADN) or higher; BSN preferred
- Ability to work in a hospice care setting and collaborate with an interdisciplinary team
- Willingness to fulfill travel requirements within the assignment scope
