Responsibilities
- Manage a caseload of patients, performing comprehensive assessments and developing individualized care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing interventions to address physical, emotional, and psychosocial needs; adjust plans as needed to optimize outcomes.
- Serve as the primary point of contact for patients, families, and the care team to ensure clear communication and continuity of care.
- Collaborate with physicians, nurses, social workers, and other staff to coordinate transitions of care, referrals, and discharge planning.
- Document assessments, plans, interventions, and outcomes accurately in the medical record; ensure compliance with regulatory guidelines.
- Monitor patient progress, re-evaluate plan effectiveness, and participate in quality improvement initiatives as part of the care management process.

Requirements
- Active registered nursing license in California
- Graduation from an accredited nursing program; BSN preferred, ADN acceptable
- 1+ year of nursing experience in case management or care coordination
- Ability to work in a hospital-based travel assignment with day shift scheduling
- Strong assessment, documentation, and interdisciplinary collaboration skills
