Responsibilities
- Coordinate and deliver comprehensive end-of-life care to terminally ill patients in alignment with hospice philosophy and regulatory guidelines.
- Manage a caseload of patients, conducting thorough assessments and developing individualized care plans.
- Provide skilled nursing interventions to address physical, emotional, and psychosocial needs.
- Serve as the primary point of contact for patients, families, and the interdisciplinary hospice team to ensure seamless communication and continuity of care.
- Handle symptom management, medication administration, patient and family education, and coordination with physicians and support staff.
- Monitor and evaluate care effectiveness, document care plans and progress, and adjust as needed to meet patient goals.

Requirements
- Current registered nurse (RN) license in California (active)
- Education: Associate degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program
- Relevant clinical experience; hospice or palliative care experience is preferred
- Ability to work effectively in a hospital setting and collaborate with an interdisciplinary team
- Strong assessment, care planning, and patient/family education skills
- Current certification as required for practice and ability to meet hospital and regulatory standards
