MDS Coordinator

Come join our team and start making a difference! This position is located at Legend Healthcare and Rehabilitation in Greenville.

Hours: Full Time Licensure: Texas RN or LVN License

Responsibilities:

• Coordinate the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS)/ PPS and development of the Interdisciplinary Plan of Care
• Complete LTCMI (Long Term Care Medicare Information), or LOC (Level of Care) forms for the Medicare payment process when applicable
• Establish and maintain positive relationships with all members of the health care team

Qualifications:

• Current RN or LVN license in the state of Texas
• Electronic Health Record Experience- preferably PCC experience
• Experience with RAI process and MDS certification preferred
• Experience in a Skilled Nursing and Long Term Care facility preferred
• Experience as a MDS Coordinator preferred.
