Home Health RN Case Manager

ProHealth Home Health and Hospice

ProHealth Home Health and Hospice is seeking a Registered Nurse to administer skilled nursing care to patients in their homes on an intermittent basis, following physician orders and the plan of care. The Home Health Case Manager (RN) will perform initial evaluations, update care plans, coordinate services, and counsel patients and families, while adhering to agency policies and quality standards. The role also involves completing OASIS assessments, maintaining documentation, and collaborating #J-18808-Ljbffr
