In this remote role, you will connect with clients from the comfort of their homes while working within our Hospice Center framework. Though primarily virtual, regular visits to clients and teamwork with healthcare professionals are essential for maintaining a high standard of care. The position typically requires working Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed. Flexibility is key, as you may need to adjust your schedule to accommodate urgent client needs and ensure continued support.

Key Responsibilities\n

As a Licensed Master Social Worker at our Counseling Center, your day-to-day responsibilities will include:

• Conducting comprehensive assessments to evaluate the psychosocial needs of seniors, focusing on their emotional, mental, and social challenges.

• Developing and implementing personalized care plans in collaboration with clients and their families that address specific needs and aspirations.

• Providing counseling to assist clients and their families in navigating life transitions, offering emotional support and guidance during challenging times.

• Advocating for client rights and facilitating access to community resources, ensuring seniors receive the necessary support and services.
\n
• Coordinating with an interdisciplinary team, including nurses, caregivers, and outside services, to create a holistic support system for clients.

• Visiting clients regularly to monitor their wellbeing, addressing any safety concerns, and enhancing their quality of life through ongoing support.

• Fostering effective communication among family members involved in the care plan and mediating any conflicts that arise.

• Educating clients and families about available community resources and enrichment opportunities to promote engagement and connectivity.

• Maintaining detailed and timely documentation in electronic health record (EHR) systems, including assessments, care plans, and progress notes.

• Staying informed on current trends, practices, and regulations within social work to uphold ethical standards in all interactions.

Patient Population / Client Focus\n

Your primary focus will be seniors receiving hospice and palliative care, many of whom may be facing serious health challenges or life-altering transitions. This role requires a high level of empathy and understanding of the complex emotional and psychological issues that arise in end-of-life care settings. You will support not only the clients but also their families, helping them cope with grief and the many changes associated with the aging process.

Qualifications\n

The ideal candidate must possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience in a social work position, preferably within a community center, hospice, or geriatric setting.
