Acute Inpatient Case Manager

Must have minimum 2 years in large hospital setting, comfortable with case load of around 25 patients. Responsible for facilitating progression of care, evaluating for next appropriate level of care, and discharge planning. Adult inpatient unit, could include med/surg, cardiology, oncology, ICU, or ortho.
