Key Responsibilities: Conduct comprehensive evaluations and continuous assessments of patient needs. Exhibit expertise in essential skills for outstanding end-of-life care, including pain management and wound care. Develop, implement, and revise care plans as needed to ensure the highest quality of care. Collaborate effectively with healthcare professionals to guarantee continuity of care. Be a strong advocate for patients and their families, ensuring their voices are honored. Educate on Advance Directives and oversee home health aides with assurance. Engage actively in interdisciplinary meetings. Requirements: Reliable transportation and a valid driver’s license. Strong scheduling and management capabilities. The ability to make sound nursing judgments in diverse field settings. Proficiency in assessing patient needs and creating tailored care plans.
