Work Environment

This position is onsite, focused primarily on home care, which means you will have the unique opportunity to meet patients right where they are comfortable. The home care setting allows for personalized treatment in a familiar environment, making it easier to build rapport with your patients. Key Responsibilities

• Conduct comprehensive evaluations that include reviewing patient histories, performing specific assessments, and documenting findings to create tailored treatment plans.
• Communicate evaluation findings and treatment strategies effectively to patients and their families, fostering a collaborative approach to goal-setting and care.
• Implement treatment protocols founded on evaluation insights and adapt them based on the patient's progress and evolving healthcare standards.
• Collaborate with a multidisciplinary team, ensuring ongoing communication regarding patient conditions and necessary adjustments to care plans.
• Manage discharge planning and ensure thorough follow-up care, maintaining accurate and timely documentation of patient interactions, treatment plans, and progress notes.
• Meet productivity expectations by practicing effective time management and maintaining a consistent schedule of patient appointments.
• Engage in departmental initiatives aimed at enhancing quality of care and compliance with relevant regulations.
• Seek out continuing education opportunities to further develop your professional skills and knowledge base, serving as a mentor to students during their clinical placements.
• Promote exceptional patient experiences through adaptability and a solution-oriented mindset.

Patient Population / Client Focus

Your focus will be on providing high-quality rehabilitative services to a diverse patient population, including individuals recovering from surgery, managing chronic pain, or undergoing post-injury rehabilitation. You will build therapeutic relationships that cater specifically to their unique health challenges and personal goals, facilitating a supportive recovery process in a personalized setting.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• Active and valid DPT license in the state of Florida.
• American Heart Association's BLS for Healthcare Providers CPR certification.
• Commitment to upholding professional standards and continuous personal development.

Preferred Experience

• A minimum of 2 years of clinical experience in physical therapy, particularly in a home care or outpatient setting.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 120,000 per year, commensurate with experience, along with a comprehensive benefits package.
