About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and OASIS within the last 6 months Description: Provides coordinates and directs the provision of home nursing care according to physicians' orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits are required. Periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is between 5-8. The contractor will be assigned between 5-10 patients per day. This equates to roughly 6-8 productivity points. Types of cases include Start of Care, Wound Care, and Follow-Up Visits. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care will be addressed. Clinical staff must hit weekly productivity points of 30. Accepting every patient on the schedule, regardless of distance from previous appointments, is required. The position may require travel up to 75 miles. The average weekly travel will be around 200 miles. All certifications and experience should be current and applicable to home health care. Local candidates are accepted at the same rate. All candidates must confirm their understanding of these expectations during the application process. On-call requirements may vary. Weekend rotation may be required once a month. Candidates must be comfortable with longer drives between patients.
