Kidney Care Transition & Care Coordination Liaison

Healthmap Solutions in New York, NY, is seeking a Clinical Community Liaison to serve as the point of contact and coordinator of care handoff with external hospital providers, focusing on transition of care for chronic kidney disease members. You will partner with PRMs, QPAs and Care Navigation teams to improve the Kidney Health Management program. You will build strong cross-functional relationships with care teams, document provider meetings per NCQA standards, and ensure timely reporting to #J-18808-Ljbffr Healthmap Solutions
