Physical Therapist (PT) – Home Health Locations: Taunton, MA. Part‑Time / Per Diem Opportunities Available. Paid per visit – routine visit rates starting at $80 per visit. Our mission is to help patients regain independence and comfort in their own homes. We’re seeking dedicated Physical Therapists (PTs) to join our growing team for part‑time or per diem visits. Key Responsibilities Evaluate patients’ physical conditions, limitations, and rehabilitation needs. Develop and implement individualized treatment plans. Provide therapy in accordance with the physician’s Plan of Care. Supervise and coordinate care with PTAs and HHAs. Maintain accurate documentation and progress notes. Educate patients and caregivers on exercises and equipment use. Assess home environments and recommend modifications. Collaborate with nurses, social workers, and other healthcare providers. Submit visit notes on time while maintaining HIPAA compliance. Qualifications Licensed Physical Therapist (PT). Minimum 1 year of PT experience preferred. OASIS and WellSky (Kinser) experience preferred (training provided). Current CPR certification. Physical exam within the last 12 months. TB test within the last 12 months. Valid driver’s license.
