Job Description
We are seeking a Family Nurse Practitioner (FNP) to join our team on a fully remote, per diem basis providing telehealth care to patients throughout New York.
ResponsibilitiesConduct telehealth patient evaluations
Assess and manage acute and chronic medical conditions
Review medications and medical history
Develop appropriate treatment plans
Order referrals and services when medically necessary
Complete accurate and timely clinical documentation
Coordinate with our clinical and administrative team
Active New York State Nurse Practitioner license
Family Nurse Practitioner (FNP) certification
Medicaid enrolled and eligible to bill
Strong communication and telehealth skills
Comfortable working independently in a fast-paced environment
Fully remote
Per diem
Flexible scheduling
Established patient flow
Administrative and scheduling support provided
