Job: Occupational Therapist (OT) Home Health - Full Time or Part Time Location: Fremont, Newark, Union City Position Overview: You will evaluate patients’ health, create and implement care plans, track progress, and support the clinical team both remotely and independently in the field. Responsibilities: Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status in the patient's home. Regularly evaluate and re‑evaluate patient status. Administer treatments, monitor patient conditions, and report signs or symptoms indicating a change in patient condition to the physician, case manager, and/or director of nursing as needed. Initiate, develop, implement and revise the patient’s plan of care (POC). Address all problems identified in the POC. Provide educational information to the patient, family, or caregiver concerning knowledge deficits or needs regarding the patient’s condition. Communicate with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device. Comply with all company policies and procedures to ensure high standards of care. Requirements: Current OT license to practice in California (required). Experience in Home Health or Hospice care – 1 year required. Current California driver’s license. Ability to travel within a 15–25 mile radius on a regular basis.
