Travel Outpatient Orthopedic Physical Therapist - $2,636 per week

Job Overview MedPro Healthcare Allied Staffing is seeking a travel Physical Therapist for a travel job in Fond Du Lac, Wisconsin. Responsibilities The Physical Therapist / PT is responsible for assessment of referred patients requiring rehabilitation services, including providing direct patient care to assess medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation. The PT will establish and administer a treatment program with specific goals according to patient capacity and tolerance under physician direction. The PT must set realistic and achievable goals for patients, document and record their condition, and educate patients and families in appropriate physical therapy methods. Develops a plan of care for each physical therapy patient. Provides skilled physical therapy services / interventions in accordance with physician orders. Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions. Collaborates with all disciplines to plan and evaluate team goals for each patient. Qualifications Must hold a Doctorate degree in Physical Therapy or a Bachelor's degree in Physical Therapy and have passed the NPTE. Must have graduated from an accredited school. Minimum one year's experience. Current state license as a Registered Physical Therapist / PT or be in the process of applying for licensure. Current CPR certification.
