Outpatient Physical Therapist - Ortho & Geriatrics

Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician direction. The role involves delegating therapy services, designing functional treatment programs, engaging in patient education, and adhering to compliance and safety standards.

Responsibilities

• Conduct comprehensive evaluations of patients and utilize clinical reasoning to develop treatment plans aimed at facilitating progress towards discharge.
• Ensure accurate and timely medical record documentation, including treatment goals and quality assessments.
• Provide therapy care in compliance with physician orders and insurance approvals.
• Participate in the student program, providing a positive learning environment and supervision.
• Create and pursue an individualized professional development plan, engaging in ongoing professional growth.
• Maintain licensure in good standing and complete all state-mandated continuing education requirements.
• Contribute to organizational performance improvement initiatives and collaborate with team members to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• At least 1 year of outpatient clinical experience.
• Current CPR certification.
• Active license to practice as a Physical Therapist in the state of employment.
• Previous experience with Select Medical is a plus.

Schedule

This is a full-time position with a 5x8 hour shift schedule.
