Intake Licensed Practical Nurse (LPN) - Remote in New Hampshire

This candidate must reside in New Hampshire. This is a full time, remote position with the occasional need to travel to the New London office. The ideal candidate is self-motivated, tech-savvy and an excellent communicator. We are looking for a team player who is willing to jump in and help, is flexible, able to prioritize and stay organized. A detail oriented Licensed Practical Nurse (LPN) to join our Intake Department for 40 hours each week. This person will assist with both Homecare and Hospice referrals. Job Highlights for Intake Licensed Practical Nurse (LPN) Become an essential part of our trusted, multi-disciplinary team Work in an environment where you are heard, empowered, and respected Excellent benefits Tuition reimbursement Responsibilities for Intake Licensed Practical Nurse (LPN) Accurately enter patient referral information into the electronic medical record system Accurately enter all incoming patient care orders and notify appropriate team members as necessary Navigate multiple electronic referral software portals Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships Triage all incoming phone calls Communicate effectively with patients and patient care givers Requirements for Intake Licensed Practical Nurse (LPN) Active NH (LPN) Licensed Practical Nurse License Must reside in New Hampshire 2 years of Health Care experience Knowledge of medical terminology, familiarity with a medical office setting, and/or triage Knowledge of Home Health/Hospice Must be comfortable working in a remote environment Proof of Flu vaccination Reliable transportation Valid driver's license Automobile insurance Physical exam within the last year Two-step TB test Lake Sunapee Region VNA & Hospice is a mid-sized agency, providing home health care, hospice, palliative and personal care services to people of all ages in parts of Grafton, Merrimack, and Sullivan counties in New Hampshire.
